CONSENT, ACKNOWLEDGMENT AND RELEASE
Project: Beacon International Neighborhoods
I hereby state that I am the legal guardian of _______________________________
________________________________ ___________(name of student and address).I understand that Second Avenue School is participating in the program Beacon International Neighborhoods, working with the Concord Consortium.
I hereby grant and release to The Concord Consortium, Inc. (CC) the right to use any of my written communication, or to film, tape, photograph or record the voice of __________________________ in connection with the development of any and all educational materials relating to the above-named project (the Materials). I hereby assign all of his/her rights, title and interest in the Materials, including his/her copyright interest therein to CC.
I acknowledge that CC will be the sole owner of the copyright and all other rights in the Materials, as well as in any software, photographic slides, bibliographies, syllabi or other materials ("Related Works") based on the Materials.
I understand that I will receive no compensation under this agreement and that CC and its assignees and licensees are relying on this Consent, Acknowledgment and Release in proceeding with the development and distribution of the Materials and Related Works.
I hereby release The Concord Consortium and its employees, independent contractors, assignees, and licensees from any and all claims and actions arising out of educational use of the materials.
